When you apply for Bright Futures you are also applying for 11 other state scholarships!
1. Social Security Number – needs to match school records
2. Date of Birth – 4 digit year
3. First Name- needs to match school records
4. Middle Initial
5. Last Name – needs to match school records
6. Are you Hispanic/Latino? – Scholarship funds may be available; indicate one or more races below with which you identify- you pick
7. Gender – male or female
8. Phone Number (at least one phone number is requested)
9. Mailing Address – no commas or hyphens
10. Apartment Number
11. City – Jacksonville
12. State- Florida
13. Florida County – Duval
14. Zip Code- just need the 5 digit number
15. E-mail address – if you enter an email address this is where your logon and password will be sent
16. Is Florida your state of legal residence?- blue drop down menu gives more information if you are not sure
17. Citizenship Status- Have a green card? You are an Eligible Non-Citizen, use the blue drop down box for more information
18. Will you graduate or have graduated from a Florida high school, Home School, or with a GED? – Yes
19. If Yes, identify your school type- Public
20. Identify the Florida county where you attended school – Duval
21. Select the Florida high school you attended- use drop down menu to select your school
22. High School Graduation Date – 06/09/2017 (last date for county)
23. In the fall of 2017, I will be classified as – Freshman, first time in college
24. Select the Postsecondary location and institution you plan to attend- put any state school right now, but make sure to update by June to reflect where you will go- this is where they will send the $$$$$
25. Select the last postsecondary location and institution you attended – SKIP THIS QUESTION
26. Unweighted High School GPA– must enter at least a 2.75 to be evaluated for Bright Futures
27. Weighted High School GPA – no penalty for guessing here
28. College GPA – SKIP THIS QUESTION
29. Some State of Florida financial aid programs require test scores. Report all tests that you have taken or plan to take. OSFA will verify scores through the appropriate test agency. –check ACT, SAT, CPT
30. Hispanic American- use the drop down menu to designate if you are from Jose Marti Scholarship country
31. Dependent child or spouse of a veteran who has been classified as deceased or 100% disabled, POW or MIA- you will need the veteran’s information
32. Family affected Rosewood incident- you will need information on that ancestor
33. Authorize release of records and test scores - SAY YES IF YOU WANT BRIGHT FUTURES
34. Guilty of a felony charge- SAY NO UNLESS YOU WERE CONVICTED OF AN ADULT FELONY
